What is Hydroxo-Adeno B12 and why two forms of B12?

Hydroxo-Adeno B12 is a lozenge with 2,000 µg vitamin B12, half as hydroxocobalamin and half as adenosylcobalamin. Both are natural, methyl-free forms: hydroxocobalamin is the form most commonly found in food, which the body can flexibly convert into whatever it needs, while adenosylcobalamin is the form that is active in the mitochondria. Together they cover the two B12-dependent enzymes in the body without bringing along a methyl group, and without cyanide, which is present in the widely used cyanocobalamin. Vitamin B12 contributes to the reduction of tiredness and fatigue, normal energy-yielding metabolism, normal functioning of the nervous system and normal red blood cell formation. Seeking Health is the brand of Dr Ben Lynch, known for his work on methylation and genetic variants such as MTHFR. Who is this methyl-free B12 intended for?

Why a lozenge with 2,000 µg?

Vitamin B12 is only absorbed well in the gut with the help of intrinsic factor, and that route becomes saturated at around 1 to 2 µg. In addition, around 1 percent of a high dose is absorbed passively; at 2,000 µg that is still amply sufficient, even if the intrinsic factor route is not working well. The lozenge also allows part of the B12 to be absorbed through the oral mucosa. Excess B12 is excreted in the urine; there is no established upper limit. If you want a lower dose, break the tablet into pieces.

How do I use the lozenges?

Start the first week with half a lozenge; let it dissolve slowly in the mouth and then swallow. Then move up to 1 tablet per day if needed, preferably in the morning. Do not take it within 5 hours of going to bed, as B12 can disturb sleep in some people. A box of 60 tablets lasts 2 months at 1 per day.

How does it differ from Methyl B12 with L-Methylfolate and from a B12 injection?

Methyl B12 with L-Methylfolate contains methylcobalamin plus methylfolate: the methylated forms, for those who tolerate them well and want folate and B12 in one tablet. Hydroxo-Adeno B12 is the methyl-free variant without folate. A B12 injection (usually hydroxocobalamin) bypasses the gut entirely and is given by the GP in the case of a diagnosed deficiency; for many people a high-dose lozenge is an effective alternative for maintenance, in consultation with the doctor.

Can I react to this product?

Hydroxo- and adenosylcobalamin are usually well tolerated, better than methylcobalamin. Occasionally someone experiences some extra energy or restlessness, a mild headache or skin blemishes when starting (acne-like spots occur with high B12 doses); in that case halve the dose or take it every other day. Xylitol can have a laxative effect in larger amounts; keep products containing xylitol out of reach of dogs.

Is it suitable during pregnancy, for children or alongside medication?

Vitamin B12 is important during pregnancy and breastfeeding, especially with a plant-based diet; discuss the dose with your midwife. For children from 4 years on the advice of a practitioner, usually a quarter to half a tablet. Metformin, acid-suppressing medication and the contraceptive pill can lower B12 status; consult your doctor if you take these, and if you have complaints have your B12 level measured before you start, because supplementation affects the measurement. A food supplement is not a substitute for a varied diet and a healthy lifestyle. If you take medication, are pregnant or breastfeeding, or have a medical condition, consult your doctor or practitioner first.
